About The Position

Case Manager (CM), RN, Rehabilitation at Blanchard Valley Hospital is a full-time position within Lifepoint Rehabilitation, which is part of Lifepoint Health. This role is dedicated to making communities healthier by providing exceptional care. The Case Manager will interview and assess patients and/or their families, determine and arrange for needed services, participate in case reviews, consult with healthcare team members on treatment plans, and assist with discharge planning. This includes identifying and referring patients to appropriate resources such as transportation, housing, healthcare, and social/spiritual services. The role also involves performing Medicaid screenings and assisting patients with Safety Net applications.

Requirements

  • Current RN license in the state of OH.
  • Current CPR certification.
  • 2 years of relevant experience required.
  • Critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Responsibilities

  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes.
  • Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Performs Medicaid screenings as indicated for NHP.
  • Assists patients with Safety Net applications when needed.
