About The Position

Moorings Park is seeking a dedicated and talented Case Manager to join our team. The Private Duty RN Case Manager is responsible for following established policies and procedures and/or specific instructions from the Clinical Supervisor or Director of Clinical Services. The Case Manager is responsible for ensuring and coordinating quality and safe delivery of home health services following applicable State/CHAP requirements. Private duty services are provided in accordance with policies and procedures and Florida state/CHAP regulations.

Requirements

  • Excellent oral and written communication skills in the English language required.
  • Associate degree or bachelor’s degree in nursing from an accredited program required
  • Active Florida Registered Nurse licensure required
  • Active Florida CPR Certification is required
  • A current Florida driver’s license, clean driving record, proof of insurance and reliable transportation are required

Nice To Haves

  • Previous geriatric or home care background preferred
  • Home Health Agency preferred experience preferred
  • A minimum of two years’ supervisory experience preferred

Responsibilities

  • Performs initial assessment of the client’s needs and service requests. Obtains necessary authorizations, consents, and intake data.
  • Develops and institutes the client’s care plan at initial visit which includes: The nursing plan of care for all skilled cases. The CNA/HHA/Companion Care Plan for all non-skilled cases.
  • Initiation of the Plan of Care in consultation with the physician for skilled cases.
  • Obtains initial and interim physician’s orders and reports on progress and changes in client’s condition to physician and Director of Clinical Services or Clinical Supervisor.
  • Ensures accurate transcription and implementation of physician's orders have been recorded.
  • Develops personal service care plan for the home health aide/companion and performs reassessment and timely revision of the individualized care plan as needed.
  • Documents timely recording of assessments, interventions, and client’s response to interventions.
  • Informs the physician, when applicable, and other team members of changes in the client’s needs.
  • Performs and documents supervision of licensed practical nurses (LPN’s) and paraprofessionals, providing services to the client, as required by policy and state regulations.
  • Provides leadership and direction to ancillary personnel.
  • Provides education to the client and family regarding the disease process, self-care techniques and disease prevention strategies where appropriate.
  • Participates in quality/performance improvement measures.
  • Attends and participates in in-service programs and shows initiative with respect to increased competency levels.
  • Maintains cooperative manner towards client/family and all members of the health care team demonstrating interdisciplinary coordination.
  • Manages equipment and supplies appropriate to care.
  • Provides case management including planning, coordination of services, therapeutic interventions, and overall case supervision.
  • Performs initial courtesy visits or follow-up visits following emergency calls as directed by Clinical Supervisor and/or Director of Clinical Services.
  • Monitors documentation of discharge planning activities and ensures discharge summary is written in timely manner.
  • Maintains client confidentiality and works in compliance with HIPAA regulations and standards.
  • Works in unison with others professionally and respectfully carrying out the Mooring Park Mission and Spirit Statements.
  • Demonstrates the desire to increase client satisfaction.
  • Demonstrates the desire to improve employee/partner satisfaction.
  • Practices standard precautions.
  • Markets Home Health Agency to referral sources and residents of Moorings Park. Participates in Health/Job fairs to promote MP HHA when requested.
  • Orients new personnel to policies, procedures, and documentation requirements of the Home Health Agency as necessary.
  • Participates in the on-call rotation as directed by Director of Clinical Services/Clinical Supervisor.
