About The Position

Virginia Mason Rehabilitation, part of Lifepoint Health, is seeking a PRN Case Manager to join their team. This role is vital in making communities healthier by providing exceptional care. Requirements

• Bachelor’s degree from an accredited nursing school or Social work program.
• Basic Life Support certification is required within 30 days of hire.
• ACLS and PALS are required within six months of hire.

Nice To Haves

• One year of experience preferred
• ASLS certification preferred

Responsibilities

• Complete departmental orientation as well as initial and annual competencies.
• Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
• Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
• Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
• Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
• Perform other duties as assigned.
