Requirements
- Active Illinois RN license (required; no IL residency needed)
- 2+ years of RN clinical experience (all specialties welcome)
- Strong communication, assessment, and care coordination skills
- Associate’s Degree in Nursing

- CCM certification

Responsibilities
- Manage member care through telephonic assessments and case management
- Develop personalized care plans to improve health outcomes
- Identify risks, remove care barriers, and support complex health needs
- Coordinate with multidisciplinary teams to ensure seamless care
- Use clinical expertise and data insights to guide decision-making
