Remote RN Case Manager (Nationwide | Monday–Friday, No Weekends) Join a mission-driven team helping members achieve better health outcomes—100% remote. We’re seeking an experienced Registered Nurse (RN) Case Manager to provide telephonic and occasional face-to-face care coordination, guiding members through complex health needs with a holistic, proactive approach.

Requirements

  • Active, unrestricted RN license (Compact license preferred)
  • Willingness to obtain additional state licenses (company paid)
  • 3+ years acute care RN experience (med/surg, ICU, or post-surgical)
  • Strong assessment, communication, and critical thinking skills
  • Travel of less than 5% may be required in the event of clinical audits

Nice To Haves

  • Case management or home health experience (1+ year)
  • Compact RN licensure
  • CCM certification
  • Proficiency with Microsoft Office tools
  • Strong telephonic communication skills

Responsibilities

  • Assess, plan, and coordinate care to support members’ overall wellness
  • Use clinical judgment and data to identify risks and remove care barriers
  • Develop personalized care plans addressing complex and co-morbid conditions
  • Collaborate with multidisciplinary teams to optimize outcomes
  • Engage members through strong telephonic communication and coaching
