Requirements
- Active, unrestricted multistate RN license (in state of residence)
- Willingness to obtain additional state licenses (company-paid)
- 3+ years of clinical RN experience
- 1+ year experience using Microsoft Office and web-based systems
- Ability to work scheduled hours with occasional evening rotation
- Associate Degree in Nursing (required)
Nice To Haves
- Case management experience in an integrated model
- Bilingual (Spanish/English)
- BSN (preferred)
Responsibilities
- Conduct telephonic (and occasional in-person) assessments of member health needs
- Develop and implement individualized care plans to improve outcomes
- Identify and address medical, behavioral, and social barriers to care
- Coordinate care across programs, providers, and resources
- Review claims/history to support effective case planning
- Engage members using strong communication and motivational interviewing skills
- Participate in case conferences and collaborate with clinical teams
