Registered Nurse (RN) Case Manager – Remote – Oncology experience required Work from home from anywhere in the U.S.! We are seeking a compassionate and clinically skilled RN Case Manager to join our team and help members navigate their healthcare journey through comprehensive case management and care coordination.
Requirements
- Active, unrestricted RN license in state of residence.
- Willingness to obtain additional state licenses as needed (company-paid).
- 3+ years of acute care hospital RN experience (medical-surgical, post-surgical, ICU, or similar).
- 3–5-year acute oncology experience required
- Associate Degree in Nursing (ADN).
Nice To Haves
- Compact RN license.
- Certified oncology nurse or certified chemotherapy nurse preferred
- 1+ year of Case Management and/or Home Health experience.
- Certified Case Manager (CCM) certification.
- Proficiency with Microsoft Office (Word, Excel, PowerPoint).
- Excellent telephonic communication and member engagement skills.
- BSN preferred.
Responsibilities
- Conduct telephonic and occasional face-to-face assessments to evaluate member healthcare needs.
- Develop individualized care plans that improve health outcomes and overall wellness.
- Coordinate services, resources, and programs to support members with complex medical and social needs.
- Apply clinical expertise to identify risks, remove barriers to care, and facilitate successful transitions across the healthcare continuum.
- Partner with multidisciplinary teams to optimize member outcomes and support effective claim management.
- Engage members through strong communication and motivational interviewing techniques.
