Registered Nurse (RN) Case Manager – Remote Make a meaningful impact on member health and wellness. We are seeking an experienced RN Case Manager to provide telephonic case management support for members with complex healthcare needs. In this role, you will assess, plan, coordinate, and manage care while helping members navigate resources, improve health outcomes, and overcome barriers to care.

Requirements

Active, unrestricted RN license in state of residence
Willingness to obtain a Compact RN license (company-paid)
Minimum 5 years of clinical RN experience
Associate Degree in Nursing (ADN)
Ability to work within Monday–Friday business hours

Nice To Haves

6+ months of Case Management or Utilization Management experience
Case Management Certification (CCM or equivalent)
BSN

Responsibilities

Conduct comprehensive telephonic assessments and develop individualized care plans.
Identify health risks, barriers, and opportunities to improve member wellness.
Coordinate care, referrals, and transitions to appropriate clinical programs and resources.
Apply clinical judgment to support members with complex and co-morbid conditions.
Collaborate with multidisciplinary teams to achieve optimal member outcomes.
Maintain compliance with regulatory requirements and company policies.
