About The Position

The RN Case Manager oversees and coordinates patient care from admission through discharge to support quality, cost-effective outcomes. The role assesses patient needs, develops care plans, and collaborates with physicians, nurses, social workers, and other providers to manage care transitions. Responsibilities also include discharge planning, utilization review, and helping patients navigate insurance and healthcare systems. This is a per-diem position with PRN day shifts of eight hours at Southern Regional Medical Center in Riverdale, Georgia.

Requirements

• At least five years of work experience after graduating from an accredited school of nursing.
• Current state Registered Nurse license.
• At least one year of experience in case management, discharge planning, or nursing management.
• Basic to intermediate computer skills and knowledge.

Nice To Haves

• Five years of acute care nursing experience.
• CCM certification, or the ability to obtain it within one year.
• Knowledge of Milliman Criteria and InterQual Criteria.
• Current BCLS certificate.

Responsibilities

• Evaluate patients’ medical, psychosocial, and discharge needs upon admission and throughout their stay.
• Develop and implement individualized care plans in collaboration with the healthcare team to support effective treatment and timely interventions.
• Coordinate safe and appropriate discharge plans, including referrals to rehabilitation, home health, or long-term care facilities.
• Monitor hospital resources and services to ensure appropriate care and avoid unnecessary treatments or extended stays.
• Help patients and families understand care plans, post-discharge instructions, and available resources.
• Advocate for patient needs and preferences, ensuring care aligns with their goals and values.
