About The Position

Resolute Rehabilitation at Cornerstone Specialty Hospitals Round Rock is seeking a PRN RN Case Manager to join their team. This role is crucial in monitoring and managing patient care to ensure continuity, optimal outcomes, patient satisfaction, cost efficiency, and compliance. The unit is a 12-bed inpatient rehabilitation unit focused on helping patients recover from serious illness, injury, or surgery, with specialized services for conditions like stroke, brain injury, spinal cord injury, orthopedic trauma, neurological disorders, and amputation. Patients typically receive 15 hours of therapy per week from a multidisciplinary team. The unit is modern, equipped with advanced rehab equipment, and designed for a patient-centered environment. As a PRN RN Case Manager, you will contribute to making communities healthier by providing exceptional care.

Requirements

• Graduate of an accredited school of nursing
• RN licensure to practice to work in the state of Texas
• Basic Life Support (BLS) - to be obtained within 30 days
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction.
• Responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
• Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
