About The Position

Central Plains Center is seeking a dedicated Case Manager / QMHP to join their team. This role focuses on improving the quality of life for individuals with mental illness, intellectual disabilities, and chemical dependency by providing accessible services and resources that support individual choices and promote dignity and independence. The position involves providing curriculum-based skills training and service coordination for children and adolescents, developing curriculum, linking clients to services, and advocating for their needs. The role also includes providing crisis intervention and participating in an on-call crisis rotation.

Requirements

Must have bachelor's degree in social, behavior or human services field from an accredited university
Must be able to qualify as a Qualified Mental Health Professional (QMHP)
Must have valid TX driver's license and be insurable

Nice To Haves

1 year of experience in dealing with Children/Families having behavioral and/or emotional problems is preferred

Responsibilities

Provides curriculum based skills training and service coordination activities for children/adolescents who have been identified as eligible for services
Develop and maintain curriculum for skills training services
Provide service coordination for clients including, but not limited to linking to other appropriate services and advocating for individuals served
Provide skills training for consumers and their family members
Ensure proper billing practices are adhered to in regards to direct reimbursable services and Medicaid administration claiming
Maintains regular contact with individuals served to monitor services and identify needs
Will provide crisis intervention services to individuals with mental illness
Will be part of the on-call crisis rotation
