About The Position

The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population. This role involves reviewing inpatient admission records to ensure proper utilization of hospital resources and determination of the appropriate level of care. The Case Manager assesses and identifies discharge needs, coordinating appropriate discharge plans. They work collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes and maintain and enhance payor relationships.

Requirements

Licensure in the state where the hospital resides is preferred.
Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
Current BLS certification required.
Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

Interdisciplinary coordination of care for a designated patient population.
Perform reviews of all inpatient admission records to ensure proper utilization of hospital resources and determination of admission for appropriate level of care.
Assess and identify discharge needs and coordinate appropriate discharge plan.
Work collaboratively with the interdisciplinary team to facilitate achievement of desired financial and quality outcomes.
Maintain and enhance payor relationships.
