Case Manager - Permanent Housing The Case Manager, Permanent Housing is responsible for providing support services to help individuals remain housed and to address needs such as benefits/entitlements, activities of daily living, substance abuse and mental health services. The Case Manager conducts home visits and links the client with local services in the community, as well as coordinating group activities for their caseload. The Case Manager provides case management services to formerly homeless individuals, some with multiple disabilities, in order to help the individual transition and adjust to permanent housing.
