Housing Case Managers will work to form supportive relationships and walk alongside clients as they develop the skills to maintain housing and live independently. Housing Case Managers may assist in any of The Place’s housing programs including Permanent Supportive Housing, Rapid Rehousing or Transitional Living. Housing Case Managers collaborate with landlords, community partners, and other staff to ensure youth are supported in safe, stable living environments. Typical work hours are Monday through Friday 9 am to 5 pm. However, schedules may have to be adjusted to accommodate client needs.

Requirements

  • Bachelor’s degree in human services or related field; OR two (2) to three (3) years relevant experience.
  • At least one (1) year of experience working with youth or vulnerable populations preferred.
  • Valid driver’s license, clean driving record, and reliable transportation required to be insurable under The PLACE’s auto policy.
  • Demonstrated understanding of trauma-informed care, harm reduction, positive youth development, and housing-first principles.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient in maintaining professional boundaries and confidentiality.
  • Ability to act with integrity, professionalism, and confidentiality.
  • Excellent written and spoken communication skills.
  • Excellent interpersonal, negotiation, and conflict resolution skills.
  • Must be computer proficient and possess experience of Microsoft Office Suite (e.g. Outlook, Word, PowerPoint, and Excel).

Responsibilities

  • Conduct assessments, develop individualized case plans, and provide appropriate support services.
  • Support independent living skills, education, and employment readiness.
  • Build trusting relationships with youth to support long-term stability.
  • Maintain accurate documentation and meet compliance/reporting standards.
  • Coordinate with landlords, property managers, and service providers.
  • Ensure services align with trauma-informed care and positive youth development.
  • Travel regularly to client residences and community locations to provide case management support.
  • Transport youth to appointments, services, and other required activities using a personal vehicle when needed.
