About The Position

Northwest Senior and Disability Services is seeking a social services worker to manage a set of consumer cases. This position performs professional-level work in the provision of social services for a caseload comprised of seniors and/or adults with disabilities, with a specific focus on OPI cases.

Requirements

  • Minimum of four (4) years of equivalent combination of education and/or experience which demonstrates the knowledge, skills and abilities required.
  • Knowledge of the problems and issues confronting the consumers the agency serves.
  • Knowledge of the state and federal laws regulating the consumer’s social services.
  • Knowledge of casework methods and techniques to individual cases.
  • Ability to prepare concise and complete case records, documentation, and reports.
  • Ability to use problem-solving and decision-making skills in order to serve consumers.
  • Ability to use appropriate questioning strategies to interview consumers to determine program eligibility and service needs.
  • Ability to support the agency mission and exemplify its core values—integrity, professionalism, service, and compassion.
  • Ability to serve as a mandatory reporter of suspected cases of neglect, exploitation, and abuse of vulnerable populations as required by policy and regulation, and make the appropriate referral to a responsible agency.
  • Ability to interact and work effectively with others in a team to deliver services to consumers.
  • Demonstrated good interpersonal communication skills through written, verbal, and nonverbal communication.
  • Ability to use sound organizational skills to meet deadlines in a timely, accurate, and efficient manner.
  • Ability to speak, read, write, and understand English and follow verbal and written instruction.
  • Secure and maintain a valid Oregon driver’s license or have an acceptable alternative means of transportation.
  • Ability to attend work regularly to meet the demands of this job and to provide necessary services.
  • Ability to complete and pass a criminal background check.

Nice To Haves

  • Bachelor's degree in social sciences or any related field.
  • Work or volunteer experience working directly with consumers in social service type settings.
  • Experience working with seniors or adults with disabilities or vulnerable populations.

Responsibilities

  • Collaborate with other staff members to comprehend and meet consumer needs.
  • Assess consumers’ non-financial needs for specific social services and determine service-eligibility.
  • Develop consumer care plans in accordance with regulations, coordinating service delivery.
  • Monitor and update consumers’ care plans according to changing needs, assessing and reducing risk.
  • Provide customer service to consumers while adhering to privacy regulations and agency standards.
  • Collaborate with a broad range of public and private social service entities, interpreting laws and connecting consumers to resources.
