About The Position

The Home Health Occupational Therapy Case Manager evaluates, develops a plan of care and treats patients to maintain or restore patients to their maximum functional performance level and completes discharge planning. Provides supervision for Certified Occupational Therapist Assistants and Certified Nursing Assistants.

Requirements

  • Required Education: Associates Degree in Occupational Therapy
  • Required Experience: Two (2) years of experience as an Occupational Therapist
  • Required Certifications and Licensures: Licensed by the Virginia Board of Occupational Therapy

Nice To Haves

  • Preferred Education: Masters Degree in Occupational Therapy
  • Preferred Experience: Skilled Nursing Facility experience

Responsibilities

  • Implements action plans to achieve or exceed service excellence benchmarks
  • Evaluates and develops plan of care according to American Occupational Therapy Association (AOTA) guidelines
  • Administers occupational therapy treatments appropriate to diagnosis and to AOTA guidelines
  • Appropriately documents and synchronizes patient care according to federal, state, and local regulations
  • Meets productivity standards
  • Uses resources (e.g., time, people, supplies, etc.) cost effectively
  • Supervises Certified Occupational Therapist Assistants, Certified Nursing Assistants and students
  • Protects patients, employees, and visitors by promoting infection-control and safety policies and protocols
  • Plans, coordinates, manages, and documents the care provided to assigned patients
  • Ensures appropriate patient and/or caregiver education with utilization of applicable resources
  • Leads, communicates and collaborates with the interdisciplinary team to plan patient care
  • Adheres to National Patient Safety Goals (NPSG)
  • Participates in performance improvement initiatives affecting patient outcomes
  • Handles conflict resolution in an appropriate, timely, and consistent manner
  • Identifies personal contributions to department cascading goals
  • Practices within occupational therapy licensure guidelines
  • Provides one in-service annually to staff
  • Engages in own professional development as evidenced by self assessment and attainment of degrees, certifications, and continuing education
  • Contributes to providing a supportive environment that is conducive to learning for all students from all academic programs
  • Other duties as assigned
