About The Position

Developmental Pathways (DP) has an opportunity for a Case Manager (CM) - Non-HCBS in our Systems Navigation (SN) Department. This role will be responsible for ensuring timely response to internal and external referrals for individuals pursuing non-waiver program supports. The Non-HCBS Case Manager will also ensure initiation and implementation of initial eligibility assessments and coordination process to access non-HCBS programs. Lastly, this role will be responsible for connecting individuals to non-HCBS program providers as requested and as needed, while also assisting Intake and Enrollment duties as needed. This role offers a 4-day, 40-hour workweek, Monday-Thursday, and includes the flexibility to work remotely, as well as the opportunity to collaborate in-person at our headquarters office in Aurora, CO, and/or at off-site locations. Schedule Enjoy a 4-day, 40-hour workweek Tuesday–Friday with the flexibility to work remotely, in-office, and in the field. This position requires frequent in-person presence to support essential responsibilities such as assessments, monitoring, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas and Elbert counties) on a weekly basis to fulfill these requirements. Requirements

  • Bachelor’s Degree; or Five (5) years of experience in the field of Long-Term Services and Supports; or (LTSS), which includes Developmental Disabilities; or Some combination of education and relevant experience appropriate to the requirements of the position
  • Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
  • Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
  • Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Responsibilities

  • Ensuring timely response to internal and external referrals for individuals pursuing non-waiver program supports.
  • Initiation and implementation of initial eligibility assessments and coordination process to access non-HCBS programs.
  • Connecting individuals to non-HCBS program providers as requested and as needed.
  • Assisting Intake and Enrollment duties as needed.
  • Serves as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases.
  • Completes various assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools.
  • Provides care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies.
  • Completes accurate case documentation in appropriate tracking and databases; prioritizes workload and meets all required timelines.
  • Completes required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered.
  • Schedules, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties.
  • Ensures the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family members.
  • Maintains knowledge of state/ federal regulations, policies and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs and other community resources.
  • Maintains a professional and ethical manner in all interactions and meets performance quality, customer service, and coordination standards as assigned by department leadership; maintains appropriate boundaries.
