Under the supervision of the Behavioral Health Administrative Manager, the Case Manager plays a pivotal role in supporting clients to break the cycle of homelessness through a holistic and proactive approach. This position encompasses conducting outreach calls and intakes, crisis management, service planning, and ongoing support for clients across various stages of homelessness. The Case Manager also coordinates services at the FTOC facility, maintains current knowledge of community resources and public benefit programs, and collaborates with community-based organizations to optimize client outcomes
