About The Position

Posterity Group LLC is a veteran owned, service-disabled small business, headquartered in Rockville, MD, that specializes in federal government contracting and staffing. We are tasked with staffing The Maine Army National Guard with various roles, including Non-Clinical Case Manager. This contract is to provide help with solider readiness. The Case Manager is expected to do the following:

Performance shall mainly be at: 194 Winthrop Street, Camp Keyes Building 7, Augusta, Maine 04333, 11 Ordnance Rd., Brunswick, ME 04011; and Regional Training Institute (RTI): 289 Hildreth St., Bangor, ME 04401.
Provide population specific and age-specific care for patients from young adulthood to older veterans/groups in the senior years of life.
Manage a defined but dynamic caseload of patients in collaboration with an interdisciplinary healthcare team.
Initiate, plan, implement, evaluate, and appropriately document patient care based on identified measurable outcomes in the patient care environment.
Anticipate and respond appropriately to emergencies, coordinate interventions, and evaluate responses through critical analysis of patient systems/operational situations.
Initiate and lead individuals or groups through the educational process to optimize health. Monitor and document all applicable clinical reminders and performance measure outcomes timely and appropriately.
Use EHRM (i.e. CPRS or CERNER- Site Dependent) and Bar Code Medication Administration (BCMA) documentation in compliance with designated facility programs.
Implement the National Patient Safety Goals to support positive outcomes for the patient group.
May provide direct oversight to other staff, nursing students, and/or be assigned as a preceptor.
Supporting the rollout of EHRM in the VHA system.

Requirements

• The Case Manager is designated as key personnel and must be a Registered Nurse III (RN III).
• The RN III is required to have a minimum of two years of experience in case management and at least three years of clinical experience.
• Valid Nursing License
• Valid BLS

Nice To Haves

• A Certified Case Manager (CCM) certification is preferred for this role.
