About The Position

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Mental Health Professional (MHP) Community Support Team who demonstrates this mission and wants to work for an organization that makes a difference in the community. The Behavioral Health CST Case Manager is responsible for the provision of mobile emergency crisis intervention services and community support and engagement services. Services are provided primarily in the community, including local hospitals, other agencies, shelters, and client homes. CST Team members are expected to work with a team of service providers within the agency and throughout the social service community to ensure that all needs of clients are met.

Requirements

  • Bachelor's degree in human services field required
  • At least one year experience working with individuals with mental illness.
  • Knowledge of the DSM V- TR or most current edition & Rule 132
  • Ability to provide services in clients’ homes and in the community
  • Navigation of stairs on a regular basis
  • Position operates in a professional office environment and off-site in various community settings.
  • Role routinely uses standard office equipment such as computers phones, copiers, and filing cabinets
  • Flexibility to work evening/overnight/weekend/rotating holidays
  • Ability to work rotating on-call shifts
  • Cover non-scheduled shifts as needed
  • Transport clients and self in personal and or agency vehicle
  • Maintain minimum amount of automobile liability insurance as defined by AID’s personnel policies
  • Valid Illinois Driver's license and good driving record as defined by AID driver eligibility requirements
  • May be required to work multiple locations
  • Must acquire and maintain certifications in First Aid, CPR, Nonviolent Crisis Intervention Training, CEU’s and other relevant trainings.

  • ASSIST and SAMHSA SOAR certification preferred.
  • At least 1 year of experience in behavioral health preferred

Responsibilities

  • Attend to scheduled work hours and be flexible to meet client and program needs, as assigned by Program Manager or Director.
  • Respond to calls from shift start to shift end.
  • Respond to calls upon being dispatched
  • Act as primary to triage/delegate and absorb first calls
  • Responsible to provide independent program coverage when scheduled.
  • Provide effective individual crisis counseling
  • Consult with family members, case manager etc.
  • Use intervention tactics to reduce symptomology
  • Provide effective telephone counseling service
  • Complete and sign required documentation on a timely basis
  • CST billing within the next 2 shifts, AND prior to program billing deadlines
  • CST log to supervisor(s) at the end of each shift
  • Attend clinical supervision a minimum of once per month, and additionally as assigned.
  • Attend and positively contribute to mandatory weekly CST meetings and be a positive contributor.
  • Acquire and maintain certification in First Aid, Adult CPR, non-violent crisis intervention (CPI), Medication Training, IM+CANS.
  • Through a team approach provides direct services to individuals assigned to the CST, at least 70% off-site during times and locations most convenient to the client, in accordance with the individual’s service plan and complete all required documentation according to standards of Rule 132, payers, and accreditation bodies.
  • Provide Direct Service for clients 70% of work hours
  • Ensure that care provided integrates all facets of clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, & housing needs.
  • Must meet monthly minimum predetermined productivity standards.
  • Complete clinical assessments that meet the requirements of program funders and applicable administrative rules.
  • Complete all necessary assessments including the Intake, IMCANS, LOCUS, HRA, consents, and required documentation including billing notes in accordance with Rule 132 standards.
  • Develops and implements crisis plans and treatment plans outlining treatment goals and interventions in collaboration with the client, CST, other providers, and other parties as identified by the client, following the standards of Rule 132, payers, and accreditation bodies.
  • Conduct, and/or participate in, treatment planning meetings.
  • Develop, review and revise IP and complete corrections within the timeframe allotted
  • Complete assessment tools and necessary paperwork (Intake, LOCUS, IMCANS, HRA, consents, client rights, privacy practices, etc. per program procedure)
  • Provide direct implementation of Individual Treatment Plan for CST clients.
  • Provide direct CST services that promote the reduction of crisis episodes and increased community functioning in order to meet rehabilitative, resiliency, and recovery goals.
  • Maintain records of individuals in the CST program in accordance to Rule 132, payers, and accreditation bodies.
  • Assist CST Team Leader in conducting regular quality assurance of CST records as assigned.
  • Assure case records are properly maintained per agency procedures including scanning necessary paperwork into Cx360 per procedure
  • Complete Authorizations, Reauthorizations, and Spend-down paperwork as assigned in a timely fashion.
  • Provide effective services for clients’ individual needs and in line with client rights and the Mental Health Recovery Model
  • Maintain open and effective communication with agency and provider staff, family, and others in response to client’s identified needs, client assistance/ service needs, treatment plan follow-up, and input on adaptations needed to enhance progress toward goals.
  • Work cooperatively with staff to ensure appropriate access to services as identified during the IP process.
  • Meet with all CST clients on a regular basis depending on program and client needs.
  • Document the distribution of client medications per procedure by accurately completing documentation in cx360.
  • Recognize emergency situations and take appropriate action.
  • Contact Manager and Director per procedure.
  • Complete necessary paperwork correctly (incident reports, petitions, case notes).
  • Promote awareness of mental health, both internally and externally.
  • Fulfill other functions on the team, as needed.
