About The Position
This is a full-time, benefits-eligible position through June 30, 2027, with potential opportunities for transfer or continuing employment based on operational needs and performance. The Case Manager will coordinate service needs and collaborate with other services and agencies. They will complete assessments and determine the appropriate level of services for new members served. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. At Presidio Palms Permanent Supportive Housing (PSH), the team is dedicated to helping individuals maintain housing stability and thrive in their community, believing that recovery starts from within. The team will assist in learning and applying coping skills, linking to resources, and supporting individuals as they navigate their hopes and dreams.
Requirements
- Master’s degree in Social Sciences
- One (1) year of experience in mental health services
- Valid and current driver’s license
- Personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.
