About The Position

Waterman Canyon Post Acute is seeking a compassionate, organized, and clinically skilled Licensed Vocational Nurse (LVN) Case Manager to join our interdisciplinary team. In this role, you will help coordinate resident care, support positive clinical outcomes, facilitate communication between healthcare providers, residents, and families, and assist with discharge planning and care transitions. Requirements

  • Current and active California LVN license.
  • Minimum one year of nursing experience preferred; skilled nursing or post-acute experience strongly preferred.
  • Strong organizational, communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Proficiency with electronic medical records and Microsoft Office applications.
  • Commitment to providing exceptional resident-centered care.

Nice To Haves

  • Previous case management, care coordination, MDS, or discharge planning experience is a plus.

Responsibilities

  • Coordinate and manage resident care plans in collaboration with the interdisciplinary team.
  • Monitor resident progress toward clinical and discharge goals.
  • Facilitate communication between residents, families, physicians, nursing staff, and ancillary departments.
  • Assist with admissions, care conferences, discharge planning, and transitions of care.
  • Track and follow up on physician orders, appointments, referrals, and authorizations as needed.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Identify and address barriers to successful discharge and continuity of care.
  • Support quality improvement initiatives and resident satisfaction efforts.
  • Participate in interdisciplinary team meetings and care plan reviews.
