About The Position

The Case Manager focuses on clinical quality and resource management, aiming for patient satisfaction and positive financial outcomes. This role involves coordinating patient care across different age groups within the Singing River Health System. The Case Manager is expected to adhere to all Singing River Health System procedures and policies, as well as guidelines from accreditation organizations. Key skills required include the ability to work effectively under pressure, multitask, be self-directed, and possess strong written and oral communication, analytical and critical thinking, conflict management, team-building, and organizational skills. The Case Manager must be capable of performing numerous tasks simultaneously with minimal errors. Responsibilities include participating in daily discharge planning meetings to address barriers and ensure smooth transitions between care levels, and fostering seamless communication between inpatient and outpatient case management teams. Requirements

  • Graduate of an accredited School of Nursing.
  • Currently licensed to practice as a Registered Nurse in the state of Mississippi.
  • Must complete and maintain BCLS certification upon hire and/or transfer.
  • Must have de-escalation training completed by the end of position orientation (90 days); must have appropriate level of de-escalation training.
  • Two (2) years’ nursing experience in ambulatory or acute care that has relevance to the practice of Case Management.
  • Strong written and oral communication skills.
  • Analytical and critical thinking skills.
  • Conflict management and team-building skills.
  • Strong organizational skills.
  • Ability to perform numerous tasks simultaneously with minimal errors.
  • Ability to work effectively under pressure, multitask and be self-directed.
  • Proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (i.e., telephone, fax, etc.), MS Outlook and Word.
  • Ability to demonstrate appropriate clinical judgment and apply appropriate professional skills to a patient population of all ages.

Nice To Haves

  • Bachelor’s Degree in Nursing preferred.
  • TB certification preferred.

Responsibilities

  • Coordinate patient care to a population of different ages within the Singing River Health System.
  • Participate in daily discharge planning meetings, addressing barriers that may inhibit discharge and targeting successful transition from one level of care to the next.
  • Create a seamless line of communication between the inpatient care management team and outpatient case management team.
  • Focus on clinical quality and resource management.
  • Ensure performed duties are in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.
