This role involves coordinating patient care across different age groups within the Singing River Health System. The Case Manager is expected to adhere to all Singing River Health System procedures and policies, as well as guidelines from accreditation organizations. Key skills required include the ability to work effectively under pressure, multitask, be self-directed, and possess strong written and oral communication, analytical and critical thinking, conflict management, team-building, and organizational skills. The Case Manager must be capable of performing numerous tasks simultaneously with minimal errors. Responsibilities include participating in daily discharge planning meetings to address barriers and ensure smooth transitions between care levels, and fostering seamless communication between inpatient and outpatient case management teams.
