About The Position

This position involves providing case management and life skills education to consumers in a residential facility. The role requires screening consumers, conducting assessments, developing service plans, and connecting consumers with community resources. The Case Manager will also assist consumers in obtaining benefits, accessing services, and developing crisis action plans. A key aspect of the role is maintaining a caseload, conducting regular case management visits, and facilitating community outings and training sessions. The position also involves coordinating with various healthcare professionals, ensuring compliance with treatment plans, and managing documentation.

Requirements

  • Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences.
  • At least two (2) years of paid work experience in the behavioral health setting.
  • Valid Driver’s License with a clean driving record.

Responsibilities

  • Screen consumers referred from RMHI for residential services and housing.
  • Conduct and document all required assessments for all consumers within 14 days of admission.
  • Complete, with consumer, a Service Plan within 14 days of admission for all consumers, based on the consumer needs as documented in the assessment process.
  • Educate and link the consumer to community resources that may assist the consumer in meeting his/her needs.
  • Assist the consumer in obtaining and maintaining benefits and assist the consumer is accessing services.
  • Complete with the consumer within 14 days a realistic Crisis Action Plan, plus PCP Communications and Emergency Contact documents for all consumers.
  • Maintain a caseload of at least 8 consumers.
  • Conduct and document at least four case management visits with each consumer each month.
  • Conduct and document at least one community outing each week for all assigned residents.
  • Conduct daily living/assistance training at least three (3) times a week for all assigned residents and document.
  • Conduct education/training at least two (2) times a week for all assigned residents and document.
  • Complete Continued Stay Reviews and submit to Director Residential Services by the assigned suspense date.
  • Maintain and update required documentation as required by Policy and Procedure.
  • Maintain all required training.
  • Attend 1 hour Annual and 4 hours Risk Area Corporate Compliance Training.
  • Attend and staff consumers with attending psychiatrist at each consumer’s scheduled appointment.
  • Ensure all prescriptions or prescription changes are turned in to the pharmacy within 24 hours of the change.
  • Coordinate and attend with the consumers all visits to the Primary Care Physician and to complete any lab work ordered by Psychiatrist.
  • Assist the consumer in the development and coordination of consumer’s discharge plan.
  • Refer consumer to outpatient case manager two weeks prior to consumer’s discharge from the residential facility.
  • Coordinate daily with the residential staff and the Residential Services Coordinator.
  • Attend annual Board and Care Training provided by the East Tennessee Office of Licensure, TN Dept of MHDD.
  • Coordinate all changes to treatment and care with the consumer’s conservator, as required.
  • Provide required documentation of compliance to the Mandatory Outpatient Treatment (MOT) Coordinator for all consumers placed in the residential facility under a MOT order.
  • Immediately notify the MOT Coordinator, Center Director and Residential Services Coordinator the consumer’s name and circumstance of any non-compliance with terms of the MOT order.
