The Client Outreach and Engagement staff members will act as the triage point for LEAD in Albany, Cohoes, and Watervliet. As such, this position must ensure effective and efficient communication and collaboration between all partners involved in referring and receiving referrals and diversions for LEAD. This position will provide harm reduction-based, person-centered case management in the community to individuals referred to LEAD. Case management services may include accompanying clients to necessary services such as homeless shelters, the Department of Social Services, court appointments, probation appointments, medical services, and meeting at a client’s home or in places in the community that have been mutually agreed upon. This position may also provide, as well as case management screening, outreach, and engagement services on an as-needed basis. Case Management staff members will conduct street outreach as needed to engage referred and diverted individuals who have yet to engage with their assigned case manager. The Outreach and Engagement staff must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers in order to best serve LEAD participant. This role works under the direct supervision of the Director of LEAD.

Requirements

  • High School Diploma or equivalent (GED/TASC) required; College degree highly preferred.
  • Minimum of one (1) year of experience in homeless services; previous experience in a human services agency is a plus.
  • Must possess a clean, valid New York State Driver’s License and have held a valid license for at least one year.
  • Must successfully pass all required pre-employment background checks.
  • Proficiency in digital tools and software, including Microsoft Office Suite (Word, Excel) and Google Workspace (Gmail, Google Docs), with a strong understanding of secure internet practices.
  • Excellent written and oral communication skills, paired with a high aptitude for detailed writing.
  • Exceptional organizational, planning, and time-management capabilities with high attention to detail.
  • Strong interpersonal skills, exhibiting genuine empathy, patience, and adaptability when working with clients and staff.
  • Ability to work effectively both independently and cohesively as a collaborative team member.
  • Demonstrated capacity to perform effectively and maintain composure in a fast-paced, high-stress environment.
  • High degree of self-motivation, engagement, and the proven ability to handle sensitive client data with strict confidentiality.
  • Ability to legally operate a motor vehicle to transport clients to appointments as needed, adhering to all criteria outlined in the Revised Policy for Agency Drivers.
  • Maintains strict confidentiality regarding client Protected Health Information (PHI), utilizing full access responsibly to execute required job duties.
  • Capability to carry out the physical and organizational requirements of the job efficiently.

Responsibilities

  • Conduct outreach in community settings frequented by individuals experiencing poverty, mental health, and substance use concerns.
  • Provide initial screening, engagement, and build trusting relationships with potential/current LEAD clients and stakeholders.
  • Deliver person-centered supportive services using evidence-based practices (such as Critical Time Intervention and Motivational Interviewing).
  • Regularly update assessments, progress notes, and goal tracking in the database to share with project management.
  • Act as a dedicated client advocate across service providers and within the criminal justice system when appropriate.
  • Participate actively in regular multidisciplinary team meetings, providing case updates and engaging in creative problem-solving to support client outcomes.
  • Develop and maintain strong collaborative relationships with key LEAD partners, Albany County offices, and community providers—including mental health and SUD treatment programs, healthcare providers, shelters, detox centers, and landlords.
  • Conduct street outreach as needed to engage referred and diverted individuals who have yet to engage with their assigned case manager.
  • Additional position-related duties and responsibilities as assigned by the supervisor.
