About The Position

The Jean Tweed Centre (JTC), in partnership with the Province of Ontario’s Justice Centres, is seeking an experienced Case Manager-Justice Access & Support System Navigator for the Toronto Downtown East Justice Centre (DTE-JC), a unique community court model. The Jean Tweed Centre is a community-based organization dedicated to supporting women with substance use, mental health, and gambling concerns, employing a client-centered approach that considers the diverse factors influencing a woman's life. Justice Centres are innovative court models that integrate health, employment, education, and social services to address the root causes of crime, aiming to break the cycle of offending and enhance public safety. The DTE-JC specifically targets the underlying causes of crime for individuals facing challenges at the intersection of poverty, homelessness, mental health, and addictions. The Case Manager-Justice Access & Support System Navigator will work exclusively with adult women participants of the DTE-JC, offering support throughout and beyond their criminal court process. This role encompasses service navigation, outreach, court accompaniment, short-term counseling, group support, warm referrals, intensive case management, and internal referrals within JTC’s gender-specific continuum of care. Case management activities will involve collaborating with participants to develop needs-based service plans using trauma and violence-informed approaches, and liaising with various health and social service providers, including mental health and substance use services, medical professionals, and community social services and housing workers. The position requires active engagement with the DTE JC team and community partners.

Requirements

  • Bachelor’s Degree in Social Work, or a related discipline.
  • A combination of 3 years of work experience in the field of, Substance Use, Criminal Justice, Mental Health and/or related field.
  • Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of substance use, mental health concerns, and involvement in the criminal justice system.
  • Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship.
  • Working knowledge of trauma-informed practice, women centred principles and best practices for women and substance use.
  • Working knowledge of psycho-social rehabilitation and recovery principles.
  • Working knowledge of the criminal justice system, housing, health care and community resources in the Toronto area and related provincial resources.
  • Ability to work independently with minimal day-to-day supervision.
  • Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills.

Responsibilities

  • Establish a comprehensive, wholistic, individualized service plan with each participant based on their identified needs
  • Lead a collaborative wrap-around process with other community-based organizations and service providers to ensure comprehensive service coordination that addresses individual needs.
  • Provide intensive case management support including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc.
  • Provide linkages to the wrap around services at Jean Tweed such as Nurse Practitioner support, the Child Development Centre, intensive live-in as well day treatment and online programs.
  • Provide linkages to long term supports including mental health and justice permanent housing
  • Actively engage with the DTE JC “team” including the Ministry of the Attorney General as well as other multi-sector community partners including Mental health, Substance Use and Justice system participants
  • Actively assist in the development of gender-responsive, and trauma informed programming for the female participants of the court
  • Complete required case notes, evaluations, and data entry reports in an accurate and timely manner.
  • Monitor the progress of clients and maintain documentation of relevant client interactions and interventions, and contacts with collateral supports and prepare reports and letters as necessary.
  • Participate in staff meetings, team development and supervision meetings.
  • Collaborate with supervisor to discuss job performance and establish professional goals.
  • Conduct job responsibilities with a commitment to harm reduction, trauma- informed, women-centred and anti-oppressive practices.
  • Abide by the Occupational Health and Safety Act, and work in a manner that is safe, reporting incidents immediately to direct supervisor.
  • Adhere to the policies and procedures of The Jean Tweed Centre.
  • Perform other duties as assigned in order to meet the overall goals and objectives of The Jean Tweed Centre.
