The Case Manager III provides comprehensive services to homebound elderly clients in the community through support services to maintain the client at home with a safe care plan. This role involves conducting comprehensive in-home assessments and reassessments of older adults' functional and cognitive capacities, identifying unmet needs, and determining eligibility for benefits. The Case Manager III will develop and implement care plans in collaboration with seniors and their families to address needs and ensure safe living conditions. The position also includes providing crisis intervention for emergencies, emergency coverage for meal delivery clients, and performing intake activities such as assessing eligibility and providing referrals. Maintaining accurate case notes, waiting lists, and timely submission of reports are also key responsibilities.
