About The Position

VOA Northern California & Northern Nevada is a large provider of community-based social services, dedicated to transforming lives and meeting the needs of families, individuals, veterans, seniors, and youth. As a Certified Great Place to Work, we are recruiting for a full-time Case Manager II for the Family Program at our Mather Community Campus. This role is crucial in providing compassionate care and empowering individuals to achieve emotional, physical, and spiritual well-being. The Case Manager II will work independently and assist Case Manager I, functioning at a highly skilled level in assessment, individual goal planning, group facilitation, community resource development, file monitoring, and summary development. Collaboration with the Housing Specialist and Employment Specialist is key to ensuring successful client outcomes. This position is committed to improving service quality through participant, employee, and stakeholder input, ensuring equitable service delivery and accessibility.

Requirements

  • Two years of successful experience in the delivery of human services or any combination of education, training and experience, which would provide the knowledge and ability to complete the duties outlined above.
  • Knowledge and experience with social casework objectives, principles and methods.
  • Knowledge of Child Protective Services (CPS) and Adult Protective Services (APS).
  • Experience working with the homeless population and the barriers faced by the unhoused.
  • Extensive experience with strength-based case management and trauma-informed care models.
  • Experience working with low-income families from diverse social and ethnic backgrounds who have a history of homelessness or disability.
  • Ability to maintain a high degree of confidentiality and professional integrity.
  • Ability to demonstrate excellent administrative and organizational skills.
  • Ability to demonstrate excellent written and verbal communication skills.
  • Ability to demonstrate personal effectiveness through strong problem-solving and time-management skills.
  • Proven ability to represent the interests of the organization, interact effectively with a diverse population, and be comfortable building successful collaborative relationships with families and individuals, staff and other stakeholders.
  • Understanding of public welfare laws, principles of community organization, and effective use of public and private community resources.
  • Experience and skill to establish and maintain effective individual and group relationships with participant families, employees and community partners.
  • Ability to demonstrate proficiency working with computers including but not limited to Microsoft Office Suite and database systems.
  • Ability to react under pressure, use good judgment in ambiguous situations, and be flexible/adaptable.
  • Must pass all required background clearances in accordance with agency, County, State, and federal standards.
  • CPR/First Aid certification is required within 90 days of employment.
  • Completion and maintenance of all required training and health clearances.

Responsibilities

  • Perform daily work requirements to achieve established objectives of the department.
  • Assist Case Manager I, Housing Specialist, and Employment Specialist with client barriers.
  • Perform overall assessment of program participant’s needs and develop an individualized service plan with short- and long-term goals.
  • Review case plans weekly to adjust goals where needed or appropriate.
  • Educate program participants on issues surrounding living skills, parenting, financial capability, workforce development, health and hygiene, housing, and drug and alcohol abuse.
  • Develop and maintain comprehensive lists of community resources for use by program participants; refer program participants accordingly.
  • Document all contact with program participants; prepare all screenings, assessments, service plans, evaluations, and reporting records.
  • Provide crisis intervention, referrals and collaborative consultation with any service providers working with the participant.
  • Maintain and update all participant files to ensure compliance.
  • Act as a peer review committee and conduct monthly file review of participant files.
  • Attend and conduct meetings and training, as directed.
  • Review Case Manager I client case notes/services on HMIS, to keep in compliance.
  • Assist participants with mediation, critical thinking, and problem solving.
  • Support Case Manager I with participants and caseload when needed.
  • Support effective communication between Case Managers, Employment Specialists, and Housing Specialists.
  • Establish and maintain positive relations with the general public, funding agency, participants and other staff.
  • Ensure compliance with all VOA policies, CARF standards, and safety and security regulations.
  • Perform other duties as assigned.
