About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. This is a temporary position expected to last approximately 6–8 months. The program utilizes a multidisciplinary team approach and operates under a "whatever it takes" philosophy, providing individualized, recovery-oriented services tailored to each participant's unique needs and goals.

Requirements

High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
Valid and current driver’s license
Personal vehicle insurance with your name listed as a driver
Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Responsibilities

Serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders.
Assists individuals to live as independently as possible in their chosen community.
Drives clients to appointments and groups using a personal or company vehicle.
