People Incorporated is seeking a Case Manager II to join our Waiver Case Management (St. Louis County) team. This role involves working with clients diagnosed with chronic mental illness or other ongoing health conditions. The primary focus is to connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management emphasizes behind-the-scenes service coordination. Our Case Managers meet with clients twice annually and build long-term relationships by developing dynamic service plans. Staff operate autonomously, creating their own hybrid schedules after initial training. The position offers a manageable caseload size, averaging 30 individuals, and promotes work-life balance. The role is full-time, with hours typically Monday-Friday, 8 am-4:30 pm, and is located in St. Louis County and surrounding Minnesota areas.
