About The Position

People Incorporated is seeking a Case Manager II to join our Waiver Case Management (St. Louis County) team. This role involves working with clients diagnosed with chronic mental illness or other ongoing health conditions. The primary focus is to connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management emphasizes behind-the-scenes service coordination. Our Case Managers meet with clients twice annually and build long-term relationships by developing dynamic service plans. Staff operate autonomously, creating their own hybrid schedules after initial training. The position offers a manageable caseload size, averaging 30 individuals, and promotes work-life balance. The role is full-time, with hours typically Monday-Friday, 8 am-4:30 pm, and is located in St. Louis County and surrounding Minnesota areas. Requirements

  • Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field; OR Bachelor’s Degree from an accredited four-year college with a major in any field, AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency
  • Passing all background and requirements for facility and position
  • Ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.
  • Must reside within the metro area.

Responsibilities

  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care.
  • Maintain regular contact with collaborative partners.
  • Making referrals and authorizing services based on client needs and personal outcomes.
  • Maintaining records and documenting all services provided.
  • Documenting all interventions, interactions, and relevant information to the Support Plan consistent with professional and organizational standards.
