About The Position

People Incorporated is seeking a Case Manager II to join our Targeted Case Management Metro Counties team. This role involves working with clients diagnosed with chronic mental illness, helping them connect with resources to live as independently as possible. The position supports clients in developing self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long-term relationships with their clients and operate autonomously, creating their own schedules. This is a full-time position with a hybrid work location in the Metro Counties.

Requirements

  • Passing all background and requirements for facility and position.
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.
  • Valid driver’s license, excellent driving record and current auto insurance.
  • Bachelor’s Degree in behavioral science or related field AND 2,000 hours of supervised experience working with adults with severe and persistent mental illness OR Master’s Degree in behavioral science or related field.

Responsibilities

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
