At FBH we have a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement and more. Our evidence-based, culturally appropriate, trauma informed behavioral health services address the diverse needs of those we serve. The Projects for Assistance in Transition from Homelessness (PATH) program assists individuals in accessing housing, mental health services, substance abuse treatment, disability benefits, and other services to stabilize them and facilitate recovery. Persistent and consistent outreach, and providing services at the individual’s pace, are important steps to engage people with serious mental illness who are homelessness. PATH ‘eligible’ means that an individual has a serious mental illness, or serious mental illness and substance use disorder, and is experiencing homelessness or is at imminent risk of becoming homeless. Additional services include screening and diagnostic treatment, habilitation and rehabilitation, referrals for primary health care, job training, educational services, and housing placement. Quantifiable objectives include: The number of homeless persons contacted. The percentage of contacted homeless persons with serious mental illness who become enrolled in PATH services. The percentage of enrolled homeless individuals who receive community mental health services. The number of PATH providers trained on SSI/Social Security Disability Insurance (SSDI) Outreach, Access and Recovery (SOAR) to ensure eligible homeless clients are receiving benefits. The Case Manager provides targeted outreach and case management services to adults who are experiencing serious and persistent mental disorders and homelessness. Services are focused on a population that is highly vulnerable, experiencing lack of service connections or other support, and are unable to engage in traditional service models. Services will be provided in emergency homeless shelters, transitional homeless shelters, homeless encampments, transportation hubs, train stations, and other pertinent community location where people may gather.

Requirements

  • Bachelor's degree in the behavioral sciences with two to five years related experience working with psychiatrically impaired populations OR;
  • Master's degree in the behavioral sciences and one-year experience working with psychiatrically impaired populations.
  • Must submit for Agency Affiliated Counselor Registration within 30 days of hire.
  • Must have an unrestricted and valid driver’s license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work.
  • Effective communication and interpersonal skills.

Responsibilities

  • Provide outreach and engagement services to persons with chronic mental illness who are homeless. This is inclusive of shelters, encampments and places of residence.
  • Provide consultation and training to homeless providers regarding individuals with a mental illness who are homeless.
  • Provide coordination and appropriate linkages to community resources and treatment agencies.
  • Provide stabilization and transition services to individuals with a mental illness who are homeless.
  • Coordinate with various mental health and chemical dependency treatment agencies, including DCRs, psychiatric and medical hospitals, and the criminal justice system.
  • Represent the agency in a positive and professional manner.
  • Complete required documentation and data entry, including, if required, databases outside of FBH such as the Community Management Information System (CMIS).
  • Participate in community meetings
  • Participate in Peer Review, supervision and intake and diagnostic staffing, in-FBH educational and administrative meetings as directed.
  • Maintain and enhance customer service to ensure that both external and internal customers of the FBH have a positive experience of the FBH.
  • Comply with all state WAC/RCS requirements and FBH policies and procedures.
  • Establish and maintain a good working relationship with FBH staff.
  • Perform FBH Homeless Team charting requirements, correspondence, HRIS and other paperwork in a complete and timely manner.
  • Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle.
  • Other duties may be assigned.
