Location is dependent on office availability.  Area of Focus: Behavioral Health  Program(s): PATH - Projects for Assistance in Transition from Homelessness helps adults experiencing homelessness or marginal housing and those living with significant mental illness transition to more stable housing by coordinating services that help them address their individual needs. Immediate Supervisor: Community Transitions Director -  Daniel Hyson Hours of Work: Full-time, 40 hours/week. Afterhours on-call rotation required. 

Requirements

  • High School Diploma or equivalent.
  • Qualifies as a Rehabilitative Services Associate (RSA).
  • Travels to off-site/community based locations for direct service delivery
  • Participates in after-hours crisis response rotation
  • Maintain and provide proof of a valid driver's license
  • Maintain and provide proof of personal vehicle liability insurance
  • At least 21 years of age
  • Knowledge of various behavior and human development theories and practices
  • Knowledge of local health and human services resources
  • Knowledge of basic crisis intervention and conflict resolution techniques
  • Excellent written and oral communication skills
  • Excellent computer skills

Nice To Haves

  • Bachelor’s Degree in Human Services or related field preferred
  • Qualifies as a Mental Health Professional (MHP) preferred

Responsibilities

  • Carries a caseload as assigned by direct supervisor.
  • Provides outreach and direct service to consumers in the community in their natural environment to reduce barriers to accessing services.
  • Intervenes to change attitudes, beliefs and behaviors of consumers to improve individual and family level of functioning.
  • Maintains networking and referral contacts with other human service providers both internally and externally while assessing barriers for successful engagement, treatment, and discharge.
  • Completes PATH enrollment documentation and records accurately in the electronic health record system and internal tracking systems to allow immediate supervisor to document appropriately.
  • Provides outreach services to engage homeless individuals to link with local resources for shelter, entitlements, and meeting of basic needs at a minimum of 4 hours or more per week.
  • Attends mandatory meetings and related PATH conferences or trainings as identified.
  • Provides case management services including but not limited to: advocacy, linkages to community resources, housing services.
  • Provides crisis intervention to stabilize client's condition by utilizing short term crisis intervention techniques and/or referral to other support services.
