Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. Telecare continues to advance cultural diversity, humility, equity, and inclusion at all levels of our organization by hiring mental health peers, BIPOC, LGBTQIA+, veterans, and all belief systems. New Haven Support Services provides site-based, housing-focused case management and behavioral health services to 19 individuals who reside at New Haven Court Permanent Supportive Housing development, located in Yuba City. Telecare will employ a range of housing and behavioral health interventions through a recovery-centered and Housing First approach.
