About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Forensic PATH teams will coordinate with the other elements of the Trueblood Settlement to conduct outreach in partnership with existing PATH programs and local Homeless Continuum of Care organizations to link individuals to housing, services, treatment and resources. Outreach may occur in a street outreach setting or in a service setting such as an emergency shelter, jail, encampment, or drop-in center.

Requirements

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
