Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Shifts Available: Full-Time | DAYS | Shifts: 8:00 AM - 4:30 PM | Days: Monday - Friday. New Haven Support Services provides site-based, housing-focused case management and behavioral health services to 19 individuals who reside at New Haven Court Permanent Supportive Housing development, located in Yuba City. Telecare will employ a range of housing and behavioral health interventions through a recovery-centered and Housing First approach.
