About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. CARE Court ACT will deliver time-limited, community-based, holistic support to people who meet CARE criteria and agree to participate in these services. Our person-centered approach to care meets people wherever they are in their recovery — addressing the complex needs of the individual and connecting them to resources to thrive in their community. Our program is based on the Assertive Community Treatment (ACT) model and Housing First Principles. This means that our goal is to help members become independent, productive, and integrate back into their community. We also connect members to the proper resources that can help them achieve their goals.

Requirements

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
  • Valid and current driver’s license
  • Personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

Responsibilities

  • Serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders.
  • Assists individuals to live as independently as possible in their chosen community.
  • Drives clients to appointments and groups using a personal vehicle.
