About The Position
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. The VISTA Adult Forensic ACT (Assertive Community Treatment) program provides services to people that are severely and persistently mentally ill and incarcerated within the past year. The program currently serves 30 forensic adults over the age of 18.
Requirements
- High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
- Valid and current driver’s license
- Personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
Responsibilities
- Serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders.
- Assists individuals to live as independently as possible in their chosen community.
