Requirements

Must be computer literate and possess effective written and verbal communication skills.
Ability to work independently or with a group.
Must have a strong sense of focus and must be task-oriented and non-judgmental with a clear sense of personal boundaries.
Must have a respect for confidentiality involving both clients and employees.
Ability to work in a variety of settings with culturally diverse groups. Ability to be culturally sensitive and appropriate.
Must be able to manage personal appointments and schedule, prioritize work and meet necessary deadlines.
Bachelor’s degree in social service field from an accredited college or university preferred, or equivalent combination of education and experience.
Two years of full time professional level casework experience in a social service agency preferred.
Prior experience with homeless population required.
Knowledge and adherence to Social Work or Counselor Code of Ethics.

Nice To Haves

Experience with chronically homeless men preferred.

Responsibilities

Conduct intake with clients who meet strict program criterion and enroll via HMIS/Bridges.
Conduct assessments to identify the assistance needed and develop individualized service plans for each client.
Designing structures to assist clients in enhancing their ability to focus, make effective changes, achieve desired goals, and maximize success and fulfillment in life.
Maintain awareness of potential resources in the community and make proper referrals.
Follow through with referrals made to various agencies to ensure the reasonable outcome of the referrals.
Identify client’s eligibility to various housing, employment, substance use, nutritional assistance or other applicable programs available in the community and provide appropriate program guidelines to clients.
Meet with clients regularly to ensure goals are being met.
Accurately document case notes consistently and in great detail.
Protect all confidential information, company property and electronic data. Comply with safety rules.
Other related duties as directed.
