HealthRIGHT 360, a nonprofit organization, is dedicated to providing accessible healthcare to vulnerable populations, aiming to overcome systemic barriers and promote health equity. The organization offers a broad spectrum of services, including mental health care, substance use treatment (residential and outpatient), and primary health services. They also support individuals transitioning back into the community after involvement with the criminal justice system. By integrating physical and behavioral health, HealthRIGHT 360 empowers individuals through addressing social determinants of health, fostering resilience, and facilitating recovery. The Destiny In-Custody program, in collaboration with the County, aims to reduce the likelihood of substance use relapse and criminal behavior leading to re-incarceration. The program serves male individuals with a history of justice involvement, focusing on those transitioning from jail to the community and those at risk of relapse while already in the community. The Case Manager II role involves providing comprehensive case management services to inmates in Solano County. This includes offering information, support, and follow-up services as outlined in the Re-entry Plan. The Case Manager II will address various needs such as sober living and transitional housing, stable housing, employment, entitlement benefits, family reunification, mental health and SUD treatment, childcare, vocational training, rent subsidies, deposit assistance, civil legal services, gender-specific mentoring, clean slate services, and subsidized employment and transportation.
