This position assumes responsibility for delivering direct client case management services to Permanent Supportive Housing residents using a Housing First approach, and in the context of an Integrated Primary and Behavioral Health Team model. The role involves conducting bio-psychosocial assessments, assisting individuals and families with developing strengths-based mental health care plans, monitoring client service needs, and assisting clients in applying for benefits and community support services. The Case Manager I is responsible for documenting all client encounters in an electronic record and service log within 24 hours, facilitating weekly psychosocial and educational groups, and maintaining monthly contact with each person on their caseload. Additionally, the role may involve transporting clients to appointments and attending weekly Blended Management Meetings.
