About The Position

The Case Manager I ensures the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Mental Health for contracted agencies as well as adhering to the mission of St. Joseph Center. The position provides case management services (individual rehabilitation, skill building) to adults diagnosed with severe and persistent mental illness and substance use disorders and works within a multi-disciplinary team.

Requirements

  • Must be highly motivated and a self-starter.
  • The ability to communicate with and relate to a diverse group of people including clients, community, and other staff.
  • Must have excellent organizational skills and the capability to work in a fast-paced environment.
  • Strong knowledge of homeless services and resources.
  • Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
  • Ability to use widely supported internet browsers.
  • Access to reliable transportation.
  • Travel to meet with clients as needed.
  • Travel to meetings at multiple St. Joseph Center sites and at community partner locations as needed.

Responsibilities

  • Provide case management and supportive services to meet the needs of each individual.
  • Build trust and rapport with individuals.
  • Provide linkage to physical health, substance abuse, meaningful community activities, volunteer services, job placements etc.
  • Provide support with individual rehabilitation and skill building.
  • Link clients to appropriate housing and assist them in completing required paperwork to obtain appropriate permanent housing.
  • Develop and maintain strong collaborative relationships with other service providers involved in the client’s care.
  • Collaborate and consult regularly with FSP team’s interdisciplinary team members.
  • Conduct ongoing case management assessments identifying client barriers to achieving their individual goals.
  • Identify and utilize client strengths to increase self-efficacy and independence.
  • Provide ongoing intensive case management services that ensure successful permanent housing, decrease social isolation, decrease/prevent relapse risks, advocacy to deter evictions, and linkage and ensure smooth transition into permanent housing by supporting clients with completing benefits and housing application processes, life skills such as budgeting and cooking, accessing community resources, and other skills as needed.
  • Provide crisis management such as assessing suicidal/homicidal ideation when necessary.
  • Communicate with landlords and other collaborating agencies, responding to urgent housing requests in a timely manner.
  • Participate in meetings with collaborative agencies and other community meetings as directed.
  • Complete and maintain all documentation necessary by DMH requirements, agency requirements and program requirements.
