About The Position
Adelphoi is expanding its Family Finding Program and is looking for a dedicated individual to join the team in Northumberland County as a part of the Family Finding service. Family Finding establishes a lifetime network of support for children and youth who are disconnected or at risk of placement outside of their home and community. The Family Finding process identifies family members and supportive adults estranged from or unknown to the child, especially those who are willing to become permanent connections for them.
Requirements
- BA/BS in Social Work or a related Human Service field
- Minimum of 2 years of experience in the childcare field
Responsibilities
- Implement six clear goals and activities for any child/youth enrolled in the service: Discovery, Engagement, Planning, Decision Making, Evaluation, and Follow up on supports.
- Searching for connections for the youth.
- Facilitating casework sessions to assess for support, and safety needs of youth.
- Maintaining regular contact with the youth, their support resources and the referring agency.
- Documenting the process from Discovery to finding supports and connections.
