As a Case Manager, Housing (Site Based), you will be among the first team members helping shape the culture, services, and participant experience within this new community. This is more than a case management role—it's an opportunity to help build a program from the ground up while supporting individuals on some of the most important journeys of their lives. You'll work directly with residents to develop goals, strengthen stability, access resources, and create pathways to long-term housing success. Through relationship-based, person-centered services, you'll help individuals achieve greater independence while ensuring they feel seen, valued, and supported along the way.
