About The Position

This position is open to current Colorado residents only and operates under a hybrid working arrangement, requiring a minimum of 2 days in the office and a maximum of 3 days remote work. Work arrangements are subject to change. Located within the Colorado Department of Human Services, the Office of Civil and Forensic Mental Health (OCFMH) oversees the public behavioral health system, including community programs, two mental health hospitals, and the Forensic Services Division. The Forensic Services Division is responsible for various units, including Forensic Community-Based Services (FCBS), Jail-Based Evaluation and Restoration (JBR), and others. This division carries out statutory obligations to supervise individuals adjudicated Not Guilty by Reason of Insanity (NGRI) who are released into the community under conditional release or placement. The division also provides oversight, care coordination, and treatment for individuals committed to CDHS custody but residing in the community. Our teams work in diverse settings such as community agencies, jails, courts, and hospitals, including secure facilities. Clients may be unpredictable due to mental health needs, requiring staff to make critical decisions in dynamic environments to ensure accountability for patient care. Staff must maintain updated skills and competencies through continuing education and clinical supervision. Travel throughout Colorado may be required.

Requirements

  • Six (6) years of relevant experience in an occupation related to the work assigned to this position OR A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years.
  • Must use state vehicles to conduct in-person case management visits and attend meetings as needed.
  • Must be available for emergency calls.
  • Must have a FULL BACKGROUND CHECK.
  • Must possess a valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border. Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.

Nice To Haves

Responsibilities

  • Overseeing clients adjudicated as Not Guilty by Reason of Insanity (NGRI) who are court-ordered into the custody and care of the FCBS.
  • Contributing to care coordination and case management for clients with psychiatric and legal needs.
  • Monitoring compliance with court-ordered conditions.
  • Supporting and facilitating resource referrals and acquisition.
  • Partnering with community providers to ensure adequate care and compliance.
  • Serving as a liaison with internal and external stakeholders.
  • Maintaining strong knowledge of Colorado statutes, risk and mitigation factors, community resources, and complex system processes.
  • Assisting with smooth and safe transitions into the community, supporting maintenance of stability, ongoing community treatment engagement, and clients’ interactions with the legal system.
  • Managing a caseload of FCBS clients, including in-reach efforts with hospital teams to establish rapport and understand individual needs.
  • Monitoring ongoing psychiatric stability, safety, and support needs of clients living in the community under court-ordered conditions.
  • Monitoring and supporting adherence to agreements and court-ordered conditions, and communicating changes or violations.
  • Serving as a primary liaison and consultant with community stakeholders.
  • Supporting resource referral needs in partnership with treatment team members.
  • Engaging client support networks.
  • Offering case management services.
  • Attending staffings.
  • Participating in home visits.
  • Facilitating administration, collection, and submission of forensic tests (e.g., urinalysis, hair test) and receiving results to inform care needs.
  • Contributing to plans of care.
  • Monitoring condition compliance (e.g., housing, employment, medication, treatments).
  • Monitoring ancillary individual needs such as finances, food, clothing, and transportation.
  • Engaging in property and home searches when required by conditions.
  • Partnering with treatment teams to facilitate referrals for evaluations and changes in conditions.
  • Drafting documentation and entering regular contact notes in electronic health record platforms in accordance with HIPAA.
  • Regularly documenting client contact notes within the internal electronic health record system in a timely, thorough, and accurate manner.
  • Completing legal documentation for submission to the courts, including quarterly reports, annual reports, probable cause memorandums for violations, assisting with drafting probable cause affidavits, drafting 'Requirements for Community Placement', drafting conditional release orders, and other FCBS-related court communications and reports.
  • May engage as a liaison for judges, attorneys, guardians, attorneys general, and others affiliated with the criminal justice system.
  • Fielding questions and concerns via email, telephone, meetings, and training.
  • Providing testimony when required on behalf of the Forensic Services Division.
  • May be required to contact law enforcement.
  • Performing any other duties that may arise while completing the tasks of this position to meet the needs of the unit, the Department, and the Division.
