About The Position

Necco is a social enterprise dedicated to transforming child welfare and is seeking purpose-driven professionals to join its community. The Foster Care Case Manager role involves working with children and foster parents to help children achieve their goals, supporting the growth and development of foster families, and providing essential resources for permanency. This position requires working with clients in various settings including offices, communities, schools, and foster homes.

Requirements

21 years of age
Current LSW or LPC or Bachelor's degree in a human services-related field (OH, KY, GA)
Bachelor's degree with field experience of 2 years (WV)
Bachelor's degree or Master's degree from an accredited four-year college or university (Tennessee)
Degree supplemented by at least 7 hours in behavioral sciences (social work, psychology, sociology)
One year of casework experience can substitute for the 7 hours in behavioral sciences
Minimum of 2 years' experience working with children with emotional or behavioral problems
Limited Liability Auto Insurance Coverage of 100/300/100
Valid Driver's License
Organizational skills
Written/Oral Communications skills
Skills necessary to navigate and execute several software programs (e.g., Microsoft Office, Databases, Electronic Health Records, Dashboards)
Successful completion of all required Criminal Background checks and drug screenings per state and company regulations
Ability to adhere to scheduled and unplanned deadlines
Willingness to travel and attend training
Successful completion of all required training pertaining to the job

Responsibilities

Participate in the referral matching process for children and families, advocating for the best possible home match.
Facilitate and develop each child’s Initial Treatment Plan and Comprehensive Treatment Plan.
Facilitate and/or ensure completion of appropriate Assessments (e.g., Biopsychosocial, Independent Living, Diagnostic).
Refer foster children to applicable support services (e.g., behavioral health, independent living).
Ensure initial legal documentation is obtained prior to placement or service delivery.
Ensure appropriate safety plans are developed during intake.
Develop and facilitate each consumer’s Individualized Treatment Plan (ITP).
Assist foster parents and foster children with the implementation, tracking, and monitoring of the Comprehensive Treatment Plan.
Establish relationships with foster parents and foster children through training and support.
Meet and/or exceed all state and licensure regulations regarding contact with foster children on caseload.
Ensure foster children receive all required medical and therapy services as outlined in the service plan.
Serve as a liaison between Necco and placement agencies and community resource entities.
Participate in service meetings with various stakeholders.
Establish relationships with school and education personnel to ensure children meet educational goals.
Provide ongoing training and support to foster parents and children.
Monitor and review documentation to ensure compliance with state, licensure, and accreditation requirements.
Maintain confidentiality as outlined in policy.
Provide input and/or assist team members in Corrective Action Plans (CAP) for foster parents and execute assigned tasks.
Monitor and review medication logs for foster parent compliance with medication administration.
Ensure discharge planning includes referral to aftercare services.
Complete home visits as required by rules.
Deliver services to clients across multiple environments, including in-home and community-based settings.
Transport clients as needed and travel between locations.
Participate in a weekly rotating "on-call" schedule for advisory and support.
Document and report on-call activities and follow up with team leaders.
Report instances of non-compliance, abuse, neglect, or suicidal ideations to supervisors or designated authorities immediately.
Ensure all critical incidents are staffed with team leaders and entered into the Electronic Health Record within established timeframes.
Complete and submit Medicaid documentation as appropriate.
Ensure complete and accurate data is entered into the Electronic Health Record (e.g., ECEM, General Contact, Academic supports, medical information).
Transfer established/stable caseloads and foster homes to new Case Managers and mentor them.
Support teammates to ensure all foster children receive Necco standard services.
Ensure foster parent and case manager needs are met during staff transition periods.
Attend and participate in self-directed tactical meetings and activities.
Perform other duties and responsibilities as assigned by supervisor.

Ensure all employee-related documents are current and up to date for the employee personnel file.
