Requirements

  • High School diploma or GED
  • Two years of providing psychiatric rehabilitation services to a disabled population/low income and/or homeless population (preferred)
  • Bachelor’s degree in Social Work or related field (preferred)
  • Valid class C driver’s license
  • Access to a vehicle for transportation
  • Proof of valid automobile insurance
  • Ability to be insured by MHALA liability insurance
  • Knowledge of health care and social service resource systems
  • Knowledge of self-help intervention strategies
  • Crisis intervention and conflict resolution skills
  • Proficiency in Motivational Interviewing, Harm Reduction approach, and Trauma-Informed Care
  • Strong oral and written communication skills

Responsibilities

  • Provide housing placement, support, and case management services for eligible homeless veteran families through outreach, assessment, and engagement.
  • Assist veterans and their families in finding and accessing community resources using intensive case management based on housing first and psychosocial rehabilitation models.
  • Interface with government, community, and private agencies to secure rental subsidies, loans, or other resources for transitional and permanent housing.
  • Assist and coach members through the application process for housing and benefits.
  • Provide follow-up during the application process and act as a housing counselor and advocate.
  • Conduct presentations on MHA's housing programs and work with landlords to create housing opportunities.
  • Provide or arrange necessary services for members moving into new housing using Motivational Interviewing and Harm Reduction practices.
  • Conduct regular home visits, provide in-home case management, and make community-based referrals to ensure successful housing.
  • Act as a liaison with landlords, service providers, or other tenants to resolve issues.
  • Represent MHALA at community housing provider meetings, HUD-VASH, and LAHSA meetings.
  • Stay current with changes in housing programs and requirements.
  • Network with other veteran and mainstream service agencies to share resources and develop programs.
  • Attend staff meetings and participate in agency operations.
  • Collaborate with staff to ensure seamless delivery of supportive services.
  • Assist with program documentation standards and submit required data collection documents within deadlines.
  • Collaborate with members to establish working budgets for housing sustainability.
  • Mediate and advocate on behalf of members to help them obtain and maintain housing.
