We are seeking a full-time, Case Manager for our Lowell Day Program. What does a Case Manager do? Responsible for maintaining Guest Confidential Files in compliance with established requirements. Responsible for writing grammatically sound ISP and DHSP goals, objectives and implementation strategies that focus on skill attainment. Writes comprehensive and grammatically sound progress notes and assessments in accordance with the timing requirements set forth by funding sources. Responsible for developing an in-depth understanding of Guests strengths and needs and for communicating these strengths and needs in a professional manner to families/guardians, funding sources and fellow Colleagues.
