About The Position

Suncoast Center is seeking compassionate and dedicated individuals to join their team of caring professionals as a case manager to support and advocate for their clients. Suncoast Center offers a variety of programs such as intensive community based mental health, forensic programs, office-based mental health and substance use care, as well as programs designed to assist children and their families. Case managers coordinate with client’s family members and community supports to ensure client needs are met and all involved with a client’s treatment work together. Case managers are an integral part of the Clinical Care Team and require strong time management skills and flexibility. They assist clients in developing realistic effective and measurable treatment plan goals and objectives, provide direct clinical services using solution-oriented strategies, and may provide on-call crisis intervention covering nighttime and weekend hours as requested.

Requirements

  • Bachelor's degree required from an accredited college or university in relevant field.
  • Minimum 1 year of experience providing at least 12 months of related case management services to either the adult or child population.
  • Completion of AHCA approved targeted case management training within 3 months of hire.
  • Must be certified through the Florida Certification Board (FCB) within 6 months of hire.
  • Must be 21 years of age.
  • Must have a negative drug screening prior to hire and throughout employment.
  • Must pass a Criminal Background Check, local checks, and DCF Level II Background Screening prior to hire and annually.
  • Must have valid Florida Driver’s License and pass motor vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance.
  • Must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character.

Responsibilities

  • Meets with the client to accurately and rapidly assess and document the client’s symptoms, diagnosis, and interventions, and assures that all program criteria are met for opening the case.
  • Completes all documentation to open the client into TCM.
  • Completes an accurate and up to date assessment, functional assessment and service plan within 30 days of the client’s case being opened, and every six months thereafter.
  • Assists clients in developing realistic, effective and measurable care plans. Establishes prioritized behaviorally descriptive, realistic desired changes with clients, and develops a realistic plan for their accomplishment with appropriate time frames.
  • Maintains a Continuity of Care in accordance with Florida Statutes and Florida Administrative Codes, 65E-15, Case Management Regional Operating Policy and Florida Medicaid Manual.
  • Acts as an Advocate for the client at all relevant staff meetings, court hearings, etc. or arranges for a substitute.
  • Links the client with necessary resources to assist with the client’s needs and monitors the effectiveness of these interventions.
  • Uses appropriate crisis intervention skills and anticipates and prevents crisis where possible.
  • Provides services in a way that utilizes the principles of recovery, and treats every client as a partner in a process to better their lives.
  • Remains current in the general knowledge of psychotropic medication, side effects, and DSM IV diagnosis, and recognizes when medical services are appropriate.
  • Makes a minimum of one monthly face to face contact, and one bi-monthly home visit, with all clients or documents reasons for failure.
  • Meets productivity standard of at least 80%.
  • Utilizes support funds effectively to meet the needs of the clients.
  • Completes all progress notes and other documentation (i.e. time sheets, outcome measures, FARS etc.).
  • Complete and maintain documentation in a timely and accurate manner.
  • Promotes communication and collaboration among agency programs.
  • Flexibility to provide services in client homes and other community settings.
  • Ability to be flexible with hours to meet client scheduling needs
