About The Position

Case Manager - Community Supports (CS) & Enhanced Care Management (ECM) Full-Time Youth for Change is a non-profit, public benefit organization licensed by the state of California to provide comprehensive treatment, education, and support services for our community. Our mission statement is “Enhancing the well-being of children, individuals, families and communities” and we have successfully done this for over 30 years. The Case Manager for Community Supports (CS) & Enhanced Care Management (ECM) works as a member of the Youth for Change team and supports several programs within the agency. This position is a community-based, whole-person care role. As a professional case manager for CS and ECM this position will promote independence and well-being, conduct outreach, evaluate needs, and connect vulnerable Medi-Cal/ CalAIM populations with integrated physical, behavioral, social services, and making changes through skill development.

Requirements

Bachelor's Degree in Public Health, Human, Social Welfare or related field; or equivalent experience, training and coursework.
Minimum of one year of progressively responsible and direct work experience working with outreach and engagement, assessments, care planning, resource coordination, housing navigation, documentation and compliance among other experience.
Strong understanding of community resources and services
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Experience working with diverse populations including homeless individuals
Strong organizational and time-management skills
Possess a valid California Driver’s License with a clear driving record for the past three (3) years.
